Comprehensive network of support advocacy, referral, and intervention to adults being discharged from mental healthcare facilities.
Our Integrated Case Management Services (ICMS) is an assertive outreach program which emphasizes assessment, advocacy, empowerment, referral, linkage, and supportive counseling. Services are designed to assist adults in their recovery based on individual needs and interests and helping them gain access to medical, social, educational, housing and other services and resources available to them.
ICMS services are provided predominantly off-site in the consumer’s natural environment (“in-vivo”) and have no out of pocket cost to the individual.
Primary goals include (1) Engage and provide referrals, linkages and support; (2) Enable a smooth transition through all phases of illness and recovery; (3) Empower persons with mental illness to independently manage their own lives in the way they choose; (4) Address the specific needs and assist in service procurement, delivery, coordination, and integration.
ICMS services include but are not limited to Community/ In home outreach; Systems Advocacy; Coordination and monitoring of service delivery; Medication and illness education to individuals and their families; Service panning with an emphasis on personal wellness.
Individuals referred to ICMS must be 18 years or older, resident of Essex, Morris, or Passaic County, and have a diagnosis of a psychotic disorder, major depressive, or bipolar disorders for at least 12 months.

The goal of In Home Therapy for Seniors is to help consumers regain their mental health and enter into productive lives as individuals, family members, and members of society. Consumers shall receive psychotherapy services appropriate to their needs in order to improve the quality of their lives. When indicated, consumers in need of psychotropic medications will be referred accordingly. Treatment includes individual and family therapy. Consumers are evaluated solely based on their ability to utilize and benefit from psychotherapy. Most of the treatment is conducted by psychotherapists in the consumer’s natural environment.
